Preparing for Your First Visit

To help us provide the best possible care, please:

• Complete New Patient Forms: Fill out the necessary forms prior to your appointment. These can be accessed and submitted through our Patient Portal 
• Bring Identification and Insurance Information: Please bring a valid photo ID and your insurance card to your appointment.
• List of Medications: Provide a current list of medications, including prescriptions, over-the-counter drugs, and supplements.
• Medical History: Be prepared to discuss your medical history, including any previous dermatological treatments or conditions.

Insurance and Billing

We accept a variety of insurance plans and strive to make the billing process as straightforward as possible. 

• Co-Payments: Co-pays are due at the time of service.
• Prior Authorizations: If your insurance requires prior authorization or referrals for certain procedures, please ensure these are obtained before your visit.
• Self-Pay Patients: If you do not have insurance or choose to self-pay, payment is due at the time of service.
