Preparing for Your First Visit

To help us provide the best possible care, please:

Complete New Patient Forms: Fill out the necessary forms prior to your appointment. These can be accessed and submitted through our Patient Portal
Bring Identification and Insurance Information: Please bring a valid photo ID and your insurance card to your appointment.
List of Medications: Provide a current list of medications, including prescriptions, over-the-counter drugs, and supplements.
Medical History: Be prepared to discuss your medical history, including any previous dermatological treatments or conditions.

Insurance and Billing

We accept a variety of insurance plans and strive to make the billing process as straightforward as possible.

Co-Payments: Co-pays are due at the time of service.
Prior Authorizations: If your insurance requires prior authorization or referrals for certain procedures, please ensure these are obtained before your visit.
Self-Pay Patients: If you do not have insurance or choose to self-pay, payment is due at the time of service.

Dr Montgomery is the best. When I first consulted with her regarding a couple of skin issues, I had previous appointments with 3 other Dr’s. They all missed sending in a biopsy to see if they were melanomas . Dr Montgomery did and they were. She was very thorough and my scars are very slight, important when they in visible places. She is very caring and makes you fell so protected, as she takes time to thoroughly inspect every part of my body. I have had other issues, since then, and am so grateful to be under her care. Her new clinic is lovely, very zen..including the background music. The best part is Dr Montgomery, herself.
